Mend the Gap Community Services (Mend the Gap) is a community-driven organization dedicated to breaking down barriers and ensuring fair access to the resources, supports, and opportunities people need to heal, grow, and thrive. Meet Our Leaders
Kourtney Horseman BSW RSW
My name is Kourtney Horseman, and I am a Registered Social Worker currently pursuing my Master’s degree in Clinical Social Work with a trauma-informed focus. My professional background includes mental health and wellbeing, addiction support, and work as a Certified Death Doula. I am committed to empowering individuals and take a collaborative, client-centered approach to supporting people in achieving their goals and improving their overall wellbeing.
Guided by an entrepreneurial mindset and a commitment to meaningful, community-focused change, I bring a strong ability to work alongside underserved and vulnerable populations. Since 2014, I have also developed extensive knowledge of the Residential Tenancies Act (RTA), further enhancing my capacity to advocate for individuals navigating housing-related challenges.
Rachel Young
My name is Rachel Young, and I completed my schooling as a Community Support Worker. With over five years of experience in the recovery community, I am deeply committed to supporting individuals on their path to stability and wellbeing. For more than two years, I have served as the Chair of a nonprofit organization in the Peace Region, where I have led strategic initiatives, overseen program development, and facilitated activities—including a dedicated women’s support meetings.
My extensive experience in the nonprofit sector has strengthened my ability to develop impactful programs, foster community engagement, and drive meaningful change. I am passionate about creating inclusive, supportive environments that empower individuals to achieve their goals and lead fulfilling lives.
Our programs
Our programs are designed to meet people where they are—physically, emotionally, and mentally—through tailored workshops, mobile outreach, and community-driven support. We bring resources and services directly into communities, including remote and underserved areas, and develop every initiative in partnership with those we work alongside to ensure it is culturally informed, accessible, and responsive to each person or groups unique needs.

Career & Education exploration
We support individuals in discovering their strengths, interests, and long-term goals through career exploration, education planning, and access to training programs. Our team provides guidance with applications, documentation, and navigating opportunities to help people build confident pathways toward meaningful employment or continued learning.
Skills development
Our skills development supports focus on building the tools needed for daily living and long-term stability. This includes life skills training, financial literacy and budgeting support, emotional and mental health support, addiction support, mentorship, and access to community groups. We work one-on-one and through group sessions to develop practical, empowering skills for everyday success.
Tailored Program Creation
Every workshop, program, and support service we offer is customized to the unique needs of the individuals or communities we serve. From pre-planned workshops to fully tailored programming, we design content collaboratively to ensure relevance, accessibility, cultural alignment, and meaningful impact.
Community Outreach
Our outreach team brings support directly into the community—including remote and underserved areas—ensuring that services are accessible to all. Outreach may include resource navigation, advocacy, accompaniment to appointments, basic home support, respite, and more. We are committed to ensuring equitable access for remote, underserved, and vulnerable populations, offering compassionate and practical support to promote wellbeing and stability.
